Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with primary care or internal medicine experience, preferably geriatrics, to join our team serving residents in nursing homes and assisted living communities. You will deliver personalized, coordinated health care that improves quality of life and reduces hospital visits.

You will work with supervising physicians and RN case managers to implement our national model of care in compliance with regulations.

Qualifications

• Masters degree or higher in nursing or a related field.
• APRN or PA licensure and national certification preferred.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• MS Office (Word/Outlook/Excel) skills.

Responsibilities

• Assess and manage medical and mental health needs of residents with proactive plans.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications, order labs, and coordinate diagnostics.
• Monitor treatment responses and adjust plans as needed.
• Collaborate with RN case managers as part of the care team.
