Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably with geriatrics focus. This position works to improve outcomes for long-term care residents by collaborating with a team of providers and case managers in nursing homes or assisted living communities.

You will exercise independent judgement to treat patients with acute and chronic conditions, with supervision as

Qualifications

• APRN or PA licensure required.
• EHR experience and proficiency with MS Office are preferred.
• Three years of geriatric clinical nursing or rehab experience is desirable.

Responsibilities

• Assess patients’ medical and mental health needs and create a proactive care plan.
• Ensure compliance with applicable local, state, and federal regulations.
• Prescribe medications and order labs/diagnostics as needed.
• Monitor treatment response and adjust plans accordingly.
• Collaborate with RN case managers and care team.
