Job summary

TruHealth in Columbus, NE seeks an advanced practice provider (NP or PA) with geriatrics focus to join our long-term care team. You will deliver personalized, coordinated primary care within nursing homes and assisted living communities, improving outcomes and quality of life for residents.

You’ll work under collaboration with physicians and RN case managers, handle prescribing, orders, and care plans, and navigate federal and state regulations.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• EHR experience and MS Office proficiency.

Responsibilities

• Assess residents' medical and mental health needs and create care plans.
• Prescribe medications and order labs, diagnostics, and consultations.
• Coordinate with RN case managers and supervising physicians.
• Ensure compliance with local, state, and federal regulations.
