Job summary

TruHealth seeks an advanced practice provider (NP or PA) with geriatrics experience to improve outcomes for long-term care residents, working with a team of providers and case managers. You’ll contribute to a national model of care within nursing facilities enrolled in Medicare Advantage plans, supported by a strong benefit/pay package and clear career growth.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years' experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and create a proactive care plan.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with RN case managers as part of the care team.
