Job summary

TruHealth is seeking an advanced practice provider, either an NP or PA, with experience in primary care or internal medicine, preferably geriatrics, to improve outcomes for long-term care residents.

You will work in nursing homes or assisted living communities, provide tailored care plans, collaborate with RN case managers and supervising physicians, and help implement our national model of care while ensuring compliance with state and federal rules.

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and develop a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
