Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatric focus to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers to deliver proactive, coordinated care in nursing homes or assisted living communities.

Ideal candidates hold an APRN or PA license, have a master’s or higher, and at least 3 years of geriatric clinical experience. You’ll work under federal/state guidelines and contribute to a national care model that

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a care plan that proactively manages their conditions and addresses barriers to care.
• Ensure compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to treatment and modify those plans.
• Work with RN case managers as part of an integrated care team.
