Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably with geriatrics focus. This position gives you the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers.

Qualifications

• Masters or higher in nursing or related field.
• APRN (FNP/AGNP/AHACNP) or PA licensure required.
• 3 years of clinical nursing or rehab in geriatric populations.
• Experience with electronic health records is required.
• Proficiency with Word, Outlook and Excel preferred.

Responsibilities

• Assess patients' medical and mental health needs and craft a proactive care plan.
• Ensure regulatory compliance for clinical services provided.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor treatment effectiveness and adjust plans as needed.
• Collaborate with RN case managers within an integrated care team.
