Job summary

TruHealth is hiring an advanced practice provider (NP or PA) with geriatrics experience to improve long-term care outcomes in nursing homes or assisted living settings. You will deliver personalized care plans, coordinate with case managers, and operate within TruHealth's national care model.

The role emphasizes collaboration with facility partners and health plan leadership to enhance residents' quality of life while adhering to regulations and best practices.

Qualifications

• APRN/PA license required (FNP/AGNP/AHACNP or PA)
• Degree from an accredited APRN/PA program
• 3 years' experience in geriatric clinical nursing or rehab
• Electronic Health Records experience
• Working knowledge of Microsoft applications (Word/Outlook/Excel)

Responsibilities

• Assess patients' medical and mental health needs and create a proactive care plan.
• Ensure compliance with local, state, and federal regulations governing clinical services.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers and other care team members within an integrated care model.
