Job summary

TruHealth is seeking an advanced practice provider, either an NP or PA, with geriatric-focused experience to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers, offering personalized, coordinated care that promotes quality of life.

The role involves independent clinical judgment, working mainly in nursing homes or assisted living communities, and close coordination with health plan leadership to implement our care model in compliance

Qualifications

• Licensed APRN/PA required with geriatrics focus.
• Master’s degree from an accredited APRN or PA program.
• Minimum 3 years’ clinical nursing or rehab experience in geriatric populations.
• Experience with Electronic Health Records.

Responsibilities

• Assess and manage medical plans for long-term care residents.
• Collaborate with RN case managers and providers.
• Prescribe medications and order labs and consults.
• Monitor treatment response and adjust plans as needed.
• Ensure compliance with relevant regulations.
