Job summary

TruHealth is hiring an advanced practice provider (NP or PA) to deliver primary care for long-term care residents, collaborating with a team of clinicians and case managers. You will focus on improving outcomes and patient quality of life within nursing homes or assisted living facilities.

The role emphasizes independent judgment with physician supervision, compliance with regulations, and opportunities for career growth. A strong geriatrics background is preferred.

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from accredited APRN or PA program.
• 3 years’ experience in geriatric clinical care.
• Electronic Health Records experience.
• MS Office proficiency (Word, Outlook, Excel).

Responsibilities

• Assess patients and plan proactive care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consults.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated team.
