TruHealth is hiring an advanced practice provider (NP or PA) with a geriatric focus to improve outcomes for residents in long-term care. You will work with a team of clinicians and care managers to deliver personalized, coordinated healthcare.

Qualifications

• Must hold APRN or PA license and relevant credentials.
• Degree from an accredited APRN or PA program is required.
• Minimum 3 years in geriatric clinical nursing or rehab; experience with long-term care setting preferred.

Responsabilités

• Assess patients’ medical and mental health needs and create a plan of care.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
