Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics focus to improve outcomes for residents in long-term care settings. You will collaborate within a team of providers and case managers to deliver coordinated, high-quality care.

Qualifications

• APRN/NP or PA license required.
• Master's degree or higher in APRN/PA program.
• 3 years' clinical nursing or rehab experience in geriatrics.
• Experience with Electronic Health Records.
• Proficiency with Microsoft Office (Word, Outlook, Excel).

Responsibilities

• Assess patients’ medical and mental health needs and plan care.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment response and modify care plans accordingly.
• Collaborate with RN case managers as part of an integrated team.
• Ensure compliance with local, state, and federal regulations.
