Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric focus to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers in nursing homes or assisted living communities.

You will exercise independent judgment in treating acute and chronic conditions, with supervision as appropriate, and work with health plan leadership to implement TruHealth’s national care model while ensuring compliance with regulations.

Qualifications

• APRN/PA license required and APRN/PA degree.
• Geriatric experience and primary care/ internal medicine focus.
• MS Office and EHR proficiency required.

Responsibilities

• Assess and manage residents' medical needs within geriatric population.
• Coordinate with RN case managers and physicians.
• Prescribe medications and order labs as appropriate.
• Ensure compliance with local, state, and federal regulations.
• Support training of facility staff when needed.
