Job summary

TruHealth is seeking an advanced practice provider (APRN/NP or PA) with experience in primary care or internal medicine, preferably geriatrics, to join our team in long-term care settings. You will collaborate with physicians, nurses, and case managers to deliver personalized, coordinated care that improves quality of life for residents and reduces unnecessary hospital visits.

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in geriatric clinical nursing or rehab.
• EHR experience.
• Proficient in Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and create a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and consultations.
• Monitor patient progress and adjust treatment plans.
• Collaborate with RN case managers as part of the care team.
