Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You will work with a team of providers and case managers to deliver personalized, coordinated care in nursing homes or assisted living communities enrolled in special Medicare Advantage plans.

Qualifications

Masters degree or higher in nursing or related field.
APRN or PA license required.
3 years’ experience in geriatric clinical nursing or rehab.
Electronic Health Records experience.
Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

Assess patients and create proactive care plans.
Ensure compliance with local, state, and federal guidelines.
Prescribe medications and order labs and consultations.
Monitor treatment and adjust plans.
Collaborate with RN case managers as part of an integrated care team.
