Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with a geriatrics focus to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers within nursing homes or assisted living communities enrolled in Medicare Advantage plans.

You will exercise independent judgement to treat acute and chronic conditions, consult with a supervising physician as needed, and work with facility partners and health plan leadership to implement our model of

Qualifications

• This role requires a Masters-level degree or higher in a relevant field.
• APRN/NP or PA license is required and board eligibility preferred.
• Minimum 3 years' experience in geriatric care or long-term care settings.
• Experience with Electronic Health Records and Microsoft Office applications.

Responsibilities

• Assess patients’ medical and mental health needs and develop care plans.
• Prescribe medications and order labs, diagnostics, and referrals as appropriate.
• Collaborate with RN case managers to coordinate care within the team.
• Ensure compliance with local, state, and federal regulations governing clinical services.
• Monitor patient progress and adapt plans as needed in a multidisciplinary setting.
