TruHealth is seeking an advanced practice provider (APRN or PA) with experience in primary care or internal medicine, preferably geriatrics, to work in nursing homes or assisted living communities. You will deliver personalized, coordinated health care to plan members to improve quality of life and reduce hospital visits.

Qualifikationen

• APRN or PA license is required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Experience with Electronic Health Records.

• Assess patients’ medical and mental health needs and develop a care plan.
• Prescribe medications and order labs and diagnostics.
• Monitor treatment progress and adjust plans as needed.
• Collaborate with RN case managers and care team.
