Job summary

TruHealth, in the United States, seeks an advanced practice provider (NP or PA) with experience in primary care or internal medicine, preferably geriatrics, to join our team caring for residents in long-term care. Qualifications

• Masters degree or higher in nursing or related field (APRN/NP/PA).
• APRN or PA license is required.
• At least 3 years of geriatric clinical nursing or rehab experience.
• Experience with Electronic Health Records.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and develop a care plan.
• Ensure compliance with local, state, and federal regulations in clinical services.
• Prescribe medications and order labs, diagnostics, and consults.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
