Job summary

TruHealth seeks an advanced practice provider (NP/PA) with geriatrics focus to improve outcomes for long-term care residents. You will work with a team of providers and case managers in nursing homes or assisted living facilities enrolled in Medicare Advantage plans.

Qualifications

• Masters degree or higher.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• EHR experience.
• Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal clinical regulations.
• Prescribe medications and order labs and consultations.
• Monitor treatment response and adjust plans.
• Collaborate with RN case managers as part of an integrated care team.
