TruHealth is seeking an advanced practice provider (NP or PA) with experience in primary care or internal medicine, preferably geriatrics, in St. Cloud, FL. You will work with a team to improve outcomes for long-term care residents enrolled in Medicare Advantage plans.

The role involves independent clinical judgement, collaboration with supervising physicians, and coordination with facility partners to implement our national care model. A strong benefits package is included.

Qualifikationen

• Masters or better.
• APRN/PA license required.
• 3 years’ experience in geriatric care.
• Electronic Health Records experience.
• MS Office proficiency (Word/Outlook/Excel).

Aufgaben

• Assess patients’ medical and mental health needs and develop a care plan.
• Prescribe medications; order labs and consultations.
• Coordinate care with RN case managers and the care team.
• Care for residents enrolled in Medicare Advantage plans in nursing homes.
• Implement TruHealth’s model of care in compliance with regulations.
