Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric experience to join our team in long-term care settings. You will collaborate with providers and case managers to improve quality of life for residents enrolled in Medicare Advantage plans, offering comprehensive, coordinated care.

You will exercise independent judgment in treating acute and chronic conditions, while working under a supervising physician and aligning with our national care model and regulations.

Qualifications

• APRN/PA license required.
• 3 years' experience in geriatric populations.
• Experience in long-term care or nursing home settings.
• Proficient with Electronic Health Records.

Responsibilities

• Assess patients' medical and mental health needs and create a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment responses and adjust plans as needed.
• Collaborate with RN case managers as part of the care team.
