Job summary

TruHealth in Oklahoma City is seeking an advanced practice provider (NP or PA) with a geriatrics focus for primary care in long-term care settings. You will collaborate with a team of providers and case managers to deliver coordinated care that improves residents' quality of life.

You’ll treat acute and chronic conditions, work with facility staff and health plan leadership, and participate in implementing TruHealth’s national care model in compliance with regulations.

Qualifications

• APRN/PA license required or eligible.
• Degree from an accredited APRN or PA program.
• Minimum 3 years’ experience in geriatrics nursing or rehab.
• Experience with Electronic Health Records.
• Proficiency in Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and develop a care plan for proactive management.
• Ensure compliance with local, state, and federal regulations governing clinical services you provide.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.

Education

FNP, AGNP, AHACNP or PA license

Degree from accredited APRN or PA program

3 years’ experience in clinical nursing or rehab in geriatric populations

Electronic Health Records experience

Working knowledge of Microsoft applications, including Word, Outlook and Excel
