Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with a geriatrics focus, to improve outcomes for long-term care residents. You will work with a team of providers and case managers within nursing homes or assisted living communities enrolled in Medicare Advantage plans.

You will independently treat patients with acute and chronic conditions, prescribe medications, order labs, and coordinate care with facility staff and health plan

Qualifications

• Masters or better in nursing or related field.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• APRN license required.

Responsibilities

• Assess patients and develop care plans addressing geriatric needs.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and consultations.
• Monitor treatment response and adjust plans.
• Collaborate with RN case managers as part of an integrated care team.
