Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine and a geriatrics focus. You’ll work primarily in nursing homes or assisted living communities, delivering coordinated, high-quality care to residents enrolled in special Medicare Advantage plans.

You will collaborate with a team of physicians, RN case managers, and facility staff to implement TruHealth’s national model of care,

Qualifications

• Masters or better.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• APRN Advanced Practice RN license required.
• Electronic Health Records experience.

Responsibilities

• Assessing and planning care for geriatric residents within nursing homes or assisted living communities.
• Prescribing medications and ordering labs, diagnostics and consultations.
• Collaborating with RN case managers and facility staff to implement care plans.
• Ensuring compliance with local, state and federal regulations related to clinical services.
