Job summary

TruHealth, part of American Health Partners, seeks an advanced practice provider (NP/PA) with geriatrics focus to improve long-term care outcomes. You will work with a team of providers and case managers to deliver coordinated care in nursing homes or assisted living communities.

You will exercise independent judgment, consult supervising physicians as needed, and collaborate with facility partners to implement our national care model compliant with federal and state regulations.

Qualifications

• Masters or higher required for APRN or PA role.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• APRN/PA license required; EHR experience expected.

Responsibilities

• Assess patients’ medical needs and plan care for proactive management.
