Job summary

TruHealth, a division of American Health Partners, is seeking an advanced practice provider (NP/PA) with geriatrics focus to join their team in a long-term care setting. You will collaborate with a multidisciplinary group to improve outcomes for residents in nursing homes and assisted living facilities.

Qualifications

• APR N or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Experience with Electronic Health Records.

Responsibilities

• Assess patients’ medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order lab work and consultations.
• Monitor patient progress and modify treatment plans as needed.
