Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with geriatrics focus, to deliver primary care in long-term care settings. You will collaborate with a team of providers to improve patient outcomes and reduce rehospitalizations.

You will operate in nursing homes and assisted living communities, exercising independent judgment with supervision as appropriate, and benefit from competitive compensation, benefits, and growth

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years' experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and develop a plan of care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patient progress and adjust treatment plans as needed.
