Job summary

TruHealth, part of American Health Partners, is seeking an advanced practice provider (NP or PA) to join our team delivering coordinated primary care in long-term care settings. You will work with residents enrolled in Medicare Advantage plans, improving quality of life and reducing hospital visits.

You will exercise independent judgement while collaborating with supervising physicians, facility partners, and health plan leadership to implement our national model of care in compliance with

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess and manage medical and mental health needs of residents.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor treatment adherence and adjust care plans.
• Collaborate with RN case managers and care team.

Education

Masters or better.
