Job summary

TruHealth seeks an advanced practice provider (NP or PA) with geriatric or internal medicine experience to improve long-term care outcomes. You will work with a team of providers and case managers to deliver personalized, coordinated care within nursing homes or assisted living communities enrolled in special Medicare Advantage plans.

Qualifications

Masters or higher in nursing or related field required.
APRN/PA license required.
3 years’ experience in geriatric care or long-term care.
Experience with Electronic Health Records (EHR).
Proficiency with Microsoft Word, Outlook and Excel.

Responsibilities

Assess patients’ medical and mental health needs and develop a plan of care.
Ensure compliance with local, state, and federal regulations.
Prescribe medications and order labs, diagnostics and consultations.
Monitor patient progress and adjust treatment plans as needed.
Collaborate with RN case managers as part of an integrated care team.
