Job summary

TruHealth in the United States is seeking an advanced practice provider (NP or PA) with primary care or internal medicine experience, preferably geriatrics, to join our team focused on improving outcomes for long-term care residents.

You will collaborate with facility partners and care teams to implement our national model of care, delivering personalized, coordinated services while adhering to federal and state regulations to enhance residents' quality of life.

Qualifications

• FNP, AGNP, AGACNP or PA license required.
• Masters or better.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care that proactively manages their conditions.
• Ensure compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribe medications and ordering lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to their treatment and modify those plans.
• Work with RN case managers as part of an integrated care team.

Degree from an accredited APRN or PA program

APRN Advanced Practice RN license
