Job summary

TruHealth, part of American Health Partners, seeks an advanced practice provider (NP or PA) with geriatrics experience to improve long-term care residents' outcomes within nursing homes and assisted living communities.

You will work with a team to deliver personalized, coordinated healthcare, applying independent judgement, and consulting a supervising physician as appropriate. This role emphasizes quality of life and reducing hospital visits.

Qualifications

• Masters or better required.
• APRN Advanced Practice RN license required.
• 3 years of clinical nursing or rehab in geriatric populations preferred.
• Experience with Electronic Health Records.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, procedures and consultations.
• Monitor patients’ response to treatment and modify plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
