Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine and a geriatrics focus. This role offers the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers, and by applying our national model of care.

You will work primarily within nursing homes or assisted living communities, providing personalized, coordinated health

Qualifications

• Masters degree or higher and APRN/PA license.
• 3 years of clinical nursing or rehab experience in geriatric populations.
• Experience with Electronic Health Records and basic MS Office tools.

Responsibilities

• Assess patients' medical and mental health needs and develop a proactive care plan.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment responses and adjust plans as needed.
• Collaborate with RN case managers within an integrated care team.
