Job summary

TruHealth in the United States is seeking an advanced practice provider (NP or PA) with geriatrics focus to join our team delivering primary care in nursing homes and assisted living communities enrolled in Medicare Advantage plans. You will collaborate with physicians, nurses, and case managers to provide personalized, coordinated care that improves quality of life and reduces unnecessary hospital visits.

Qualifications

• Masters or better in a related field.
• APRN Advanced Practice RN or PA licensure required.
• 3 years’ geriatric clinical nursing or rehab experience.
• Electronic Health Records experience.
• Microsoft Word, Outlook and Excel experience.

Responsibilities

• Assess patients’ medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor treatment response and modify care plans accordingly.
• Collaborate with RN case managers as part of an integrated care team.
