TruHealth is seeking an advanced practice provider (NP or PA) to join our team, focusing on primary care for aging residents in nursing homes and assisted living facilities. You will collaborate with a multidisciplinary team to provide coordinated, patient-centered care and improve outcomes while reducing hospitalizations.

Qualificações

• APRN or PA license required
• Degree from an accredited APRN or PA program
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience
• Electronic Health Records experience
• Working knowledge of Microsoft Word, Outlook and Excel

Responsabilidades

• Assess patients’ medical and mental health needs and develop plans of care
• Ensure compliance with local, state, and federal regulations for clinical services
• Prescribe medications and order lab work, diagnostics and consultations
• Monitor patient progress and adjust treatment plans as needed
