Job summary

Total Life is seeking a Nurse Practitioner for in-person longevity visits at partner senior living communities in the Merced-Fresno area. You will start with educational talks for residents, followed by one-on-one longevity visits, under the guidance of our Chief Medical Officer.

This contract, on-site role requires active CA RN license, NP certification, and experience with geriatric care and Medicare documentation. Flexible weekday site days with travel between communities are offered.

Qualifications

• Active, unrestricted CA RN license required.
• NP certification from CA BRN required.
• Active NP Furnishing Number from CA BRN.
• Current national NP board certification.
• Active DEA registration for CA; CURES registration.
• Ability to practice under standardized procedures with a physician or AB 890 NP.
• Clinical experience with adults 65+.
• Experience with Medicare documentation and E/M coding.
• Own malpractice insurance covering in-person care.
• Comfort presenting to groups and communicating with older adults and families.
• Valid driver’s license and reliable transportation.
• Ability to complete onboarding requirements (background check, TB, immunizations).
• Weekday availability for on-site days in Merced-Fresno.

Responsibilities

• Conduct 45 to 60 minute longevity visits with a geriatric assessment, medication review, cognition, mood, sleep, nutrition, mobility, fall risk, daily function, and advance care planning.
• Evaluate eligibility for GLP-1 therapy where appropriate.
• Identify residents needing follow-up care and coordinate with PCP.
• Document encounters accurately to Medicare standards in our EHR.
• Represent Total Life professionally with residents, families, and community staff.
• Travel to assigned senior living communities in Merced-Fresno area.
