Job summary

GuideStar Eldercare is seeking a Licensed Professional Clinical Counselor (LPCC) or Licensed Independent Social Worker (LISW) to provide neurobehavioral health services in long-term care facilities. You will work with a multidisciplinary team to support residents with neurocognitive disorders and psychiatric conditions.

You will evaluate residents, provide condition-specific therapy, and coordinate care plans with facilities, PCPs, and colleagues, while communicating with families as needed.

Qualifications

• Member of an interdisciplinary treatment team is required.
• Strong time management skills.
• Ability to work autonomously with regional support.
• Master’s degree in Counseling or Social Work.
• Currently licensed LPCC or LISW in your state.

Responsibilities

• Provide evaluation.
• Deliver condition-specific therapy or brief insight-based therapy as indicated.
• Coordinate care plans with the facility, PCP and GuideStar colleagues; engage family as needed.
