Job summary

GuideStar Eldercare seeks a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to deliver neurobehavioral health services in long-term care facilities. You will work with a multidisciplinary team to provide holistic care for elders with neurocognitive disorders and other neurologic or psychiatric conditions.

Qualifications

• Interdisciplinary treatment team participation is required.
• Good time management skills are essential.
• Able to function autonomously with regional Area Manager support.
• Master's degree in Mental Health Counseling or Social Work is required.
• Currently licensed as LMHC or LCSW in the state of application.

Responsibilities

• Provide evaluation of residents in long-term care facilities.
• Provide condition-specific support therapy or insight-based therapy when indicated.
• Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated.
