Job summary
GuideStar Eldercare seeks a Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) to deliver neurobehavioral health services in long-term care facilities. You will collaborate with a multidisciplinary team to ensure holistic, continuous care for elderly residents with neurocognitive disorders and other neurological or psychiatric conditions.

Qualifications
- Master’s degree in Mental Health Counseling or Social Work.
- Currently licensed LMHC or LISW in the state you are applying to.
- Ability to work as part of an interdisciplinary treatment team.
- Good time management skills.
- Able to function autonomously with regional Area Manager support.
Responsibilities
- Provide neurobehavioral health services in long-term care facilities.
- Partner with specialty clinicians to deliver holistic, continuity of care.
