Job summary

Senior Care Partners PACE is seeking a Nurse Practitioner to deliver primary medical care to frail older adults within an interdisciplinary team. Under the supervision of the Medical Director, you will build meaningful relationships with participants and families while collaborating with physicians, nurses, therapists, and social workers to coordinate high-quality, patient-centered care.

Qualifications

• MSN or NP degree required.
• Current Michigan RN license.
• Minimum of one year experience with frail elderly population.
• Strong written and verbal communication skills.
• Proficiency with MS Office and electronic systems.

Responsibilities

• Conduct comprehensive histories and physicals for new referrals.
• Perform quarterly and interval assessments for established participants.
• Evaluate and treat episodic illnesses and changing health needs.
• Participate actively in the Interdisciplinary Team (IDT) and care planning.
• Coordinate with physicians, nurses, therapists, social workers, families, and other professionals to deliver coordinated care.
• Monitor participant conditions, document changes accurately, and communicate concerns promptly.
• Develop and implement medical orders and coordinate specialty consultations as needed.
• Serve as a clinical resource to ensure continuity and quality of care.
