Senior Care Therapy, LLC is seeking a Licensed Marriage and Family Therapist (LMFT) to provide in-person psychotherapy to the geriatric population across SNFs and skilled nursing facilities in MD, NJ, NY, and PA. The role emphasizes patient-centered care with administrative support handled by SCT.

Qualifications

• Active and unrestricted LMFT license in the state applying for.
• Experience providing in-person psychotherapy to geriatric populations preferred.
• Strong documentation and regulatory compliance skills.

Responsabilités

• Conduct individual and group psychotherapy sessions in SNF/subacute facilities.
• Assess, diagnose, and treat emotional and behavioral conditions (depression, anxiety, adjustment disorders, cognitive decline).
• Develop and implement evidence-based treatment plans tailored to patients.
• Collaborate with facility staff, interdisciplinary teams, and family members.
• Maintain accurate clinical documentation using EHR systems.
• Monitor progress and adjust treatment plans as needed.
