Job summary

Optimal Care is seeking a Nurse Practitioner or Physician Assistant to deliver primary and consultative care to geriatric and medically complex patients, often in their homes, with autonomy to focus on patient-centered outcomes.

You will perform comprehensive evaluations, develop treatment plans, prescribe and monitor medications, and coordinate with physicians, caregivers, and the interdisciplinary team to improve comfort, function, and quality of life, reducing unnecessary hospitalizations.

Qualifications

• Master’s or Doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide primary and consultative care to geriatric and medically complex patients, often in homes.
• Develop and manage individualized treatment plans and coordinate with physicians and caregivers.
• Prescribe and monitor medications; ensure follow-up and care continuity.
• Coordinate with interdisciplinary teams to prevent hospitalizations and optimize quality of life.
• Travel within service area daily to deliver in-home care.
