Job summary

Attain Behavioral Health is seeking a Group Facilitator to lead Therapeutic Behavioral Services (TBS) groups in partner nursing homes. The role blends mental health support, therapeutic engagement, and relationship-building with residents who benefit from structured emotional and behavioral health programming.

Qualifications

• Must hold LSW, LPC or Ohio-recognized behavioral health credential.
• Experience facilitating behavioral health or substance-use groups.
• Experience working with older adults, long-term care, or medically complex populations preferred.
• Strong communication skills, empathy, and ability to connect with residents patiently and respectfully.
• Able to work independently in off-site facilities.
• Reliable transportation required.
• Must meet Ohio Medicaid and background check requirements.

Responsibilities

• Facilitate TBS groups for nursing home residents to support emotional health and quality of life.
• Adapt content to cognitive and emotional needs of older adults, including those with medical complexities.
• Collaborate with facility staff and on-site clinical team for consistent care.
• Complete documentation (group notes, progress updates, Medicaid paperwork) accurately and on time.
• Promote resident engagement and create a safe, respectful group environment.
• Participate in ongoing supervision and professional development.
