The position emphasizes collaboration with on-site staff, accurate documentation, and ASAM-aligned treatment planning to ensure medically necessary care for residents with complex needs.

Qualifications

• Must hold one of the following licensures: LSW, LISW, or LPC, LPCC.
• Experience facilitating behavioral health or mental health groups.
• Experience working with older adults, long-term care, or medically complex populations preferred.
• Strong communication skills, empathy, and ability to connect with residents patiently.

Responsibilities

• Facilitate TBS groups to support residents' emotional health and quality of life.
• Adapt group content for older adults with cognitive or medical needs.
• Collaborate with facility staff to ensure consistent, compassionate care.
• Complete documentation, progress notes, and Medicaid paperwork accurately.
• Conduct biopsychosocial assessments and ASAM evaluations.
• Develop individualized, medically necessary treatment plans aligned with ASAM guidelines.
