Job summary

You will review medications, resolve drug-related problems, monitor patterns across sites, and help ensure Part D compliance while supporting program outcomes. Requires NY license and ACPE degree, with geriatrics/ambulatory care experience.

Responsibilities

• Review drug information to identify therapy problems and recommend solutions to the IDT.
• Conduct comprehensive medication reviews for optimal patient outcomes.
• Monitor prescribing patterns across sites and provide education when needed.
• Audit records, ensure processes are followed and inform leadership as necessary.
• Implement policies and SOPs for medication utilization focusing on quality, safety, and cost.
• Develop patient-centered, literacy-level appropriate education for PACE participants.
• Provide in-service education to internal clinical staff on medications.
• Maintain a standardized stock medication list and tracking at each PACE site.
