Job summary
CenterLight Health System seeks a Clinical Pharmacist Case Manager to collaborate with prescribers and the IDT to optimize care for PACE participants with complex regimens. You will assess, educate, and resolve drug-related problems while supporting cost-effective medication use and adherence to Part D regulations.

Qualifications
- Graduate of an ACPE-accredited School of Pharmacy with a BS in Pharmacy or PharmD.
- PGY1 in ambulatory care or geriatrics preferred.
- New York State Pharmacy license required.
- Board certification in geriatrics or ambulatory pharmacy preferred.
- Managed care pharmacy and ambulatory care experience desired.
Responsibilities
- Review drug information and identify therapy problems with the IDT.
- Conduct comprehensive medication reviews to optimize patient care.
- Monitor prescribing patterns across sites and educate as needed.
- Audit records and report to leadership on processes and outcomes.
- Develop and implement policy related to medications with safety and cost considerations.
