Job summary

Community Physicians seeks a Patient Care Navigator to bridge clinical teams, patients, and families, ensuring smooth transitions of care across settings. You will support transitions, coordinate follow-up, and document demographics and care plans in the EMR.

Requires strong communication, bedside manner, and problem-solving; CMA preferred. This is a full-time, on-site role with comprehensive health benefits and PTO in the Deerfield, IL area.

Qualifications

• High school diploma or equivalent required.
• Graduate of an accredited Medical Assistant program is required.
• Certified Medical Assistant (CMA) is highly preferred.

Responsibilities

• Collaborate with patients, families, and multidisciplinary medical teams.
• Assist with transition clinic services, patient intake, calls, and documentation.
• Conduct patient social risk assessments to identify unmet health or social needs.
