Job summary

Community Physicians is hiring a Patient Care Navigator to bridge care among patients, families, and providers. You will support transitions, coordinate clinic and follow-up visits, and document data in the EMR.

A CMA and 1–3 years EMR experience are preferred, with 5–7 years in skilled nursing settings and strong medical terminology knowledge. You’ll travel to local facilities as needed, enjoy comprehensive benefits, and join a team focused on high-quality, patient- and family-centered care for

Qualifications

• High school diploma or equivalent required.
• Graduate of an accredited Medical Assistant program required.
• Certified Medical Assistant (CMA) preferred.
• 1–3 years of EMR/EHR experience and vital signs.
• 5–7 years in Skilled Nursing Facilities.
• Strong medical terminology knowledge.

Responsibilities

• Transition & Family Support: Collaborate with patients, families and the medical team to arrange care transitions and follow-up appointments.
• Medical Assistant & Administrative Tasks: Create charts, enter demographics, meds and diagnoses in the EMR; assist with intake and documentation.
• Assessments & Program Enrollment: Conduct social risk assessments; identify high-risk patients for transition services and enrollment.
• Interdisciplinary Collaboration: Act as a contact and advocate; attend plan-of-care meetings and communicate with practitioners.
• Outreach & Quality Metrics: Help collect key quality metrics and program performance data.
