Job summary

Community Physicians is seeking a Patient Care Navigator to bridge care transitions for older adults in skilled nursing and post-acute settings. You will coordinate with families, facility staff, and clinicians to ensure smooth transitions and follow-up appointments.

The role combines medical office tasks, EMR data entry, and advocacy, with travel to multiple facilities in the area. A CMA or MA credential is preferred; BLS is required.

Qualifications

• High school diploma or equivalent required.
• Graduate of an accredited Medical Assistant program required.
• Certified Medical Assistant (CMA) is highly preferred.
• Current Basic Life Support (BLS) Certification.

Responsibilities

• Collaborate with patients, families, and multidisciplinary medical team to support transitions.
• Assist with transition clinic services, patient intake, phone calls, and documentation.
• Conduct patient social risk assessments and identify unmet health or social needs.
• Attend plan of care meetings and communicate with practitioners and Nurse Case Managers.
• Assist in collecting quality metrics and program performance data.
