Job summary

Community Physicians is seeking a Patient Care Navigator in Illinois to bridge patients, families, and care teams across settings. You will support transitions of care, manage administrative tasks in EMR, and identify patients needing enhanced services. A CMA is preferred and BLS is recommended.

The role focuses on compassionate care for medically complex older adults in skilled nursing and post-acute settings, with a full-time schedule and comprehensive benefits.

Qualifications

• Education: High school diploma or equivalent required.
• Graduate of an accredited Medical Assistant program required.
• Certified Medical Assistant (CMA) is highly preferred.

Responsibilities

• Coordinate transitions of care across settings with patients and families.
• Perform medical administrative tasks including chart creation and data entry in the EMR.
• Identify high-risk patients and enroll them in transition services and disease management programs.
• Attend interdisciplinary team meetings and communicate with clinicians and case managers.
• Support collection of quality metrics and program performance data.
