The role requires a CMA or CMA-preferred background, strong communication, and bedside manner, with travel across local facilities. This full-time position emphasizes compassionate elder care and collaboration with interdisciplinary teams.

Qualifications

• Must have a high school diploma or equivalent.
• Graduate of an accredited Medical Assistant program is required.
• Certified Medical Assistant (CMA) is highly preferred.

Responsibilities

• Collaborate with patients, families, and multidisciplinary teams to support transitions of care.
• Perform chart creation and data entry of demographics, medications, and diagnoses in the EMR.
• Assist with transition clinic services, patient intake, calls, and documentation.
